If the logic model is about your impact's effectiveness and outcomes, the Output Tracker is about the nuts and bolts of running your social change initiative. It helps you track individual or group beneficiaries, and statistics such as number of engagements, demographics, type of assistance or training received, and the value of that.

Actually, it can track many things, and you might extend it to fit your social change initiative.
